Appointments are scheduled Monday through Friday. Our receptionists are available until 5:00 p.m. Monday through Friday to answer any questions about your appointment scheduling. Please bring any appropriate medical documents that are listed below. If you are not able to keep your appointment as scheduled or are going to be late, please contact our office as soon as possible.  We require 24 hours’ notice for surgery cancellations.

Your First Visit

Please bring your updated insurance card, a photo ID, a list of your current medications, lab results, and any questions you may have with you to your first visit. For most appointments, we ask for you to arrive 30 minutes early to complete paperwork. Please be prepared to leave a urine sample.

Follow-Up Appointments

Depending on your provider’s instructions, our amazing office staff will assist you in setting up any Follow-Up appointments. Please arrive at your Follow-Up Appointment 15 minutes early to fill out any paperwork.

If you are having a medical emergency please call 911. For questions about surgical sites, medication, or other non-life-threatening issues call our office at (970) 243-3061 to speak with a provider.
